Checking out books will be a different this year! Staff and student will fill out this Book Request Form. After we receive it, we will email you once to acknowedge receipt of your request. Then we will email you a second time when your books are ready and give you directions for pick-up.

You may request 2 books per form, so if you want more than 2 books; just fill out the form again.

Remember: You must be signed into your Google account to be able to see and use the form.
