How to Assign an Emergency Caregiver for Your Student
Sometimes, a parent/guardian may need to assign the care of a loved one to a caregiver. Whatever the situation, it’s essential to plan for the unexpected. To prepare for the possibility that you may not be able to care for your student, you may sign a Caregiver Authorization Affidavit to assign someone to make medical and educational decisions for your child in your absence.
The affidavit states who the parent/guardian wants to act as the child(ren)’s caregiver, and that the child(ren) will live with them if they are not able to live with you. It gives the caregiver the right to make decisions about your child’s education and health care for up to 2 years in your absence.
You do not give up any of your rights when you sign it. And, you can end the caregiver authorization at any time.
To create a Caregiver Authorization Affidavit, you and the selected caregiver need to appear with photo identification in front of a Notary (usually available at your bank) to complete and sign the Affidavit in the presence of the Notary, who will certify the document.
Take the following steps to make an appointment:
- Contact the selected caregiver to identify some shared dates and times that both of you can visit a Notary.
- On the date and time of the appointment:
- Both you and the caregiver must bring a form of photo identification (e.g., a license, passport).
- Both you and the caregiver are required to complete and sign the Caregiver Authorization Affidavit in the presence of a notary.
- Once the Caregiver Authorization Affidavit has been notarized, you may bring it to the Family and Community Engagement Center, inside the HYCC, 141 Bassett Ln, Hyannis, MA 02601 and they will upload this document to your child’s portal.
When the affidavit is complete, the assigned caregiver should keep the original form, and the parent/guardian will keep a copy with their important documents. The caregiver will use the affidavit if/when the child lives with them.
A copy of this form will be kept with your student’s school records. You will also want to ensure a copy is provided to the student’s doctor(s) and dentist.
