Barnstable Public School Students; Private School Students; and Student Summer Residents:
- Employment Permit Application forms (promise of employment forms) can be downloaded at www.mass.gov. Read, print, and complete the entire application (14-15 year olds must have a Doctor’s signature).
- Either email the completed application to Betty Checo or bring it to the Barnstable High School Main Office.
- School Year Office Hours: available Monday-Friday 7:00 am - 2:50 pm
- Summer Office Hours: Monday-Thursday 7:00 am - 1:00 pm (6/17/24 to 8/23/24)
- Non-Barnstable High School Students, please submit the following:
- Student name, address, hair color, eye color
- Employer name, complete address, and job title
- School name and grade of the student
- Scanned copy of the completed 2-page application with signatures
- Scanned copy of birth certificate or passport and school picture ID card
- Non-Barnstable residents residing for the summer only need to provide a written letter from the owner of the house stating they will reside there for the summer.
- Lastly, we will input the information into the computer and print the Employment Permit which will require the student’s signature (signed in the presence of the issuing agent). The student will take the permit to the employer.
