Dear Parents and
Guardians,
Please pick up your child’s medications:
Inhalers, epi-pens, or any other medication your child has in the health
office. These medications can’t be stored over the summer and will be discarded
after June 22nd. I have enclosed the new Physician’s Administration
form as well as the Parental Consent form for next if your child will be
needing medication at school. If you have any questions or concerns please stop
by the health office or call me at 508 790-6495.
