What is a School Based Health Center?
School-based health centers (SBHCs) bring the doctor's office to the school so students avoid health-related absences and get support to succeed in the classroom. At Community Health Center of Cape Cod, we work directly with your primary care provider and your insurance to provide health care to your student at Barnstable High School.

Work cooperatively within the school to become an integral part of the school. |

Provide a comprehensive range of services that meet the specific physical and behavioral health needs of the young people in the community. |

Employ a multidisciplinary team of providers to care for the students: nurse practitioners, registered nurses, mental health therapists, registered dietitians, social workers, alcohol and drug counselors, and other health professionals. |

Require parents to sign written consents for their children to receive the full scope of services provided at the SBHC. |

Have an advisory board consisting of community representatives, parents, youth, and family organizations, to provide planning and oversight. |

Where is the School Based Health Center?
We are located in the same areas as the BHS Health Services: Room 1306. A collaborative environment exists to promote health and wellness.
We believe in providing care in a nurturing, positive environment to foster the physical and mental well being of all students for optimal learning and a successful academic experience. All services are confidential except when the student is a danger to her/himself or others.
We strive to work collaboratively with the students, their family, and appropriate school staff to ensure timely and effective communication that will promote a positive healthy outcome.

Who provides the services?
The school nurse office is staffed with two registered nurses; Trish Tucker, RN, MS, FNP-C; Susan Fleming, BS, RN.
The CHCCC SBHC is staffed with a Family Nurse Practitioner, Shane Peters, APRN-BC; Maya Carpenter, Licensed Mental Health Counselor; and an Office Coordinator/Medical Associate, Soraya Prada [Certified Medical Interpreter].
We also provide:
Nutritional counseling education.
Assistance with obtaining low cost health insurance and/or a Primary Care Physician.
Cape Cod Community College, in collaboration with CHCCC, offers free Dental screenings and cleanings for identified students in need on site.
Other clinicians on site: Counselors and social workers from CHCCC and part-time Nurse Practitioners as needed.

How do I access the services?
To access the CHCCC SBHC, a signed parental/guardian consent is required for all students under 18 years of age. We work with your Primary Care Physician in the delivery of care. Visits are billed to your insurance. You are responsible for any co-payment your insurance company may. No student will be denied services for lack of health insurance.
Parents/guardians are invited and encouraged to call or visit the school health services at BHS.

Services provided by the School Nurse (s) include:

State mandated yearly screenings which include vision and hearing, height and weight and scoliosis screenings |

Screening and counseling on nutrition, eating disorders, weight management, blood pressure |

Medication administration and monitoring for students who require medications in the school setting |

Skilled nursing care, including special health care needs, and individual health care plans (IHPS) |

Monitoring of mandated immunizations per Massachusetts Department Public Health (MDPH) |

Acute and emergency care and triaging |
| - |
Health counseling and education including smoking cessation |

Crisis intervention |
| - |
Referrals and communication/collaboration with CHCCC SBHC staff, Primary Care Physician’s, dental services, parents/guardians, school support staff and community-based services. |

Assistance in obtaining health insurance and primary care physicians. |

Participation in annual sports physicals for high school and middle school students participating in school sports. (**note these physicals are for sports only and cannot be used for camp, work permits, or required school physicals. Required school physicals during high school include 10th grade, for frequent absences due to unexplained illness, transferring students, students 16 & over 14 years of age requesting employment certificates, and participation in competitive athletics prior to participation. |

Services provided by the Community Health Care Center of Cape Cod School-Based Health Center:
The Nurse Practitioner is a registered nurse with advanced skill, education, and training in a health care specialty. Working with families and with people of all ages, Nurse Practitioners provide health care similar to that provided by physicians; providing health assessments, treating illness, and prescribing medications. Nurse Practitioners also focus on wellness and prevention, giving people the information they need to make their informed decisions about their health care.

The Medical Associate arranges and schedules appointments, assists students in arranging liaisons with primary care providers, as well as other community resources and for students seeking assistance and information for medical insurance eligibility.
